Role Responsibility
You will be supporting clients in the community, by visiting them in their own homes to support well-being; encouraging activities such as drop-in activities in community centres, attending medical appointments, and encourage/ support people to engage in social inclusion activities. You will work with clients in their own homes, ensuring they are supported in taking medication, assisting them in activities of daily living, and provide emotional support, reporting any concerns to the manager of the service.
Ideal Candidate
We are ideally looking for an Outreach Support Worker with experience of working with people with autism, learning disabilities and mental health issues and a solid understanding of the issues and challenges they face.
You need to be a great team player with lots of energy, but who can also work alone. You’ll also need the strong communication skills, flexible approach and positive attitude needed to achieve targets in a challenging, changeable environment. With empathy, understanding and patience, we’ll not only expect you to love talking to people – but be passionate about helping them too. You also need to be organised, have good time management skills and feel comfortable to take initiative when needed.
Due to it being an Outreach service covering a range of areas in Chester area, the successful applicant needs to hold a full driving licence and have access to a vehicle with business insurance. You will be reimbursed for travel time and mileage.
